Gallstones often cause a pain in the upper abdomen (epigastric and under the right rib cage - i.e. "biliary colic"). The pain differs in intensity and duration, and generally occurs several hours after a meal (larger and spiced, containing meat and fat) often followed by nausea and vomiting. By biliary excretion, the body eliminates toxic waste and liver metabolites. Furthermore, the bile fluid compounds are necessary for absorption of various nutrients. The contraction of the gallbladder occurs as a result of an increased need for bile fluid excretion into the digestive system after a meal. The biliary pain is mostly caused by a gallstone stuck in the cystic duct. Sometimes it can lead to the inflammation of the gallbladder which might require an urgent surgical procedure. Patients with calcified wall of the gallbladder, verified by the ultrasound examination (porcelain gallbladder), or a gallstone larger than 2.5 cm in diameter should also undergo the prophylactic surgical procedure, due to an increased risk of the gallbladder malignant disease.

Therefore, a squirt of isotonic saline solution from the tip of the hysteroscope is sufficient for the dilatation of the cervical canal.

Under the short-lasting general anaesthesia, a thin optic instrument referred to as hysteroscope, is inserted through the vagina and the cervical canal into the uterine cavity. The hysteroscope is attached to the camera which allows a detailed examination of the cervical canal and the uterine cavity by transfering the record on the monitor. Hysteroscopy enables a direct visualization of the uterine cavity and provides diagnosis of intrauterine abnormalities by taking a sample of the uterine mucosa for the histopathological analysis (endometrial biopsy).

The transformation zone is the most sensitive part of the cervix, where “metaplasia” and HPV infection occur, being the initial stage leading to the cervical dysplasia. The procedure is performed when a cervical change does not respond to the conservative therapy methods.
What does the surgery include?
It is a minor surgical procedure, since the excision does not reach the depth of the cervix, while a lesion is removed approximately at the level 8 to 10 mm in depth. The pathohistological analysis of the tissue should verify if the lesion has been completely removed. The marginal damage of the tissue is reduced by applying higher current frequencies, so the histological analysis of the specimen margins should not pose a problem. The advantages of the electrosurgical technique over the traditional cone biopsy are simplicity of the procedure, shorter operation time, possibility of outpatient surgery, minor postoperative discomforts and earlier recovery of a patient.

There are no recommended preparations for the procedure in the first stage of the menstrual cycle, but a patient is to avoid any physical effort and should rest for the next 24 hours after the procedure. When is the surgical procedure required?
The inflammation is caused by aerobic and anaerobic bacteria, as well as Chlamydia and gonococcus, which damage the glandular excretory canal causing the blockade of the excretory pathway and accumulation of the secrete substances, turning the excretory canal and gland into a cyst or even abscess, i.e. the lesion filled with pus, in case of severe inflammation. The surgery is necessary when the female patient experiences pain, while the abscess causes sitting and walking difficulties. A larger Bartholin’s cyst can also cause discomforts and change the external genital appearance, even without the abscess formation.
What does the surgery include?
In case of the abscess of the Bartholin’s gland, it is necessary to perform the surgical procedure to cut an opening in the abscess and evacuate the pus. The aesthetic results after the complete removal of the Bartholin's gland are usually unacceptable. The recurrence of the disease is common in case of opening of the abscess and draining of the pus. Therefore, it is necessary under local or general anaesthesia to form a new, dilated excretory canal of the gland to allow constant evacuation. A vertical incision, measuring 1 to 3 cm in length, is performed through the vaginal vestibule in the area of the Bartholin’s cyst (abscess). The collection begins to evacuate spontaneously following the incision and is additionally cleaned with a physiological saline and/or hydrogen. The wall of the cyst is then everted, pulled closer to the vestibular mucosa and attached with resorptive sutures, followed by the insertion of the silicone Penrose drain which enables draining of the secretion collection.
